Outreach
Service
The
Outreach service was established five years ago as more and more
people were able to move out of The Rathbone Centre and live independently
with support workers visiting them in their own homes.
The
service now works with over 40 people, with a team of support workers
offering a variety of different types of support package tailored
to each person’s individual needs. Support visits are arranged
to fit in with the lifestyle and preference of the person.
We
work with people who live alone, some who live with their families
and some who live in other types of home situation such as supported
living.
The
service is registered with the Commission for Social Care Inspection
who oversee all services like ours and ensure that high standards
are maintained.
